You may register as a patient by filling out a new patient registration form available from reception and providing us with sight of two forms of identification such as your birth certificate, driving licence or passport and a utility bill (one must include your current address).
You will also be asked to complete a health questionnaire, and to make an appointment with one of our Practice Nurses. This information is helpful to your doctor as obtaining records from your previous practice can take some time.
Please note: Newly registered patients will need to see a doctor or nurse before any medication will be prescribed.
